*Verification of your Health Insurance Coverage is not a guarantee of payment and must be verified before your first appointment. Most of our providers our in network with specific health insurance companies and changes yearly, some may not be in network any longer and your insurance may not have updated their provider listings. You are ultimately responsible for all charges incurred that your insurance does not pay for. Please verify before booking your first appointment that we accept your health insurance. If you are a new patient using medical insurance, please book only an our listed initial or new patient service with insurance. If you do not receive a confirmation of coverage from us and you book any other service you will be charged for the non medical booked service.
